Job summary

Highmark Health is seeking an RN Case Manager in Canonsburg, PA to develop and evaluate care plans across the health continuum. You will collaborate with patients, families, and an interdisciplinary team to coordinate clinical care and advocate for optimal outcomes.

The role requires a nursing degree or equivalent experience, PA licensure (or multi-state via eNLC) and CPR certification, with a focus on ongoing professional development.

Qualifications

• Minimum Bachelor’s Degree in Nursing or Bachelor’s Degree with Nursing Diploma; or 6 years of relevant experience in lieu of a degree.
• 3 years in a clinical nursing role.
• Current State of PA RN licensure or multi-state licensure via eNLC; CPR certification; professional certification within 5 years preferred.

Responsibilities

• Contribute to or complete comprehensive assessments of patients and families/caregivers.
• Identify barriers and analyze situations for the assigned patient population.
• Coordinate clinical care with the patient, family, provider(s), and interdisciplinary team.
• Advocate for patients and families through effective communication with providers and team members.
• Educate patients and families leveraging industry standards.
• Seek professional development opportunities through certification and mentoring.
• Adhere to policies, procedures, rules, regulations, and requirements of clinical settings.
