Job summary

Allegheny Health Network is seeking an experienced Registered Nurse Case Manager to create, implement, and evaluate patient care plans. You will collaborate with clinical teams, health plans, and external partners to optimize outcomes and resource use.

The role focuses on readmissions, length of stay, ED utilization, and patient/family satisfaction. The candidate will need a BSN or a BA with nursing diploma and 3 years of clinical nursing experience, PA RN licensure or eNLC, and CPR.

Qualifications

• Minimum: Bachelor's Degree in Nursing or Nursing Diploma with 6 years of relevant experience.
• 3 years in a clinical nursing role.
• Current State of PA RN licensure or multi-state licensure via eNLC.
• Professional Certification within 5 years of start date.
• CPR certification (American Heart Association)

Responsibilities

• Contributes to or completes comprehensive assessment of patients and families, including social determinants and barriers to health management.
• Identify barriers and analyze situations to determine safe transition and care coordination.
• Coordinates clinical care with patient, family, providers, and interdisciplinary team.
• Advocates for patients and families through effective communication with providers, payers, and post-acute partners.
• Educates patients/families and team on care appropriateness, documentation, utilization, benefits, and transition planning.
• Pursues professional development through certifications, mentoring, and participation on committees.
• Adheres to hospital policies and regulatory requirements.
