Job summary

Highmark Health is seeking an experienced nurse case manager to assess and coordinate care for patients and families across the health continuum. You will collaborate with providers, health plans, and post-acute partners to optimize outcomes and resource use.

You will advocate for patients, educate families on care plans, and pursue ongoing professional development in a fast-paced hospital setting.

Qualifications

• Bachelor’s degree in Nursing required; 6 years of experience allowed in lieu of degree.
• 3 years in a clinical nursing role.
• Current PA RN licensure or multi-state licensure via eNLC.
• Strong communication and collaboration skills with diverse teams.

Responsibilities

• Contributes to comprehensive assessment of patients and families, including social determinants and barriers.
• Identifies barriers and analyzes for safe transition and care coordination.
• Coordinates clinical care with patient, family, providers, and interdisciplinary team.
• Advocates for patients with providers, payers, and post-acute partners to assure outcomes.
• Educates patients/families on care plans, documentation, insurance and resources.
• Seeks professional development and participates in committees.
• Adheres to hospital policies, procedures, and regulatory requirements.
