Qualifications

• 5 years in clinical, case/utilization management, and/or disease condition management.
• Current State RN licensure (or multi-state licensure via eNLC).
• Oncology experience is a plus and culturally competent care.
• Strong communication and advocacy skills.

Responsibilities

• Manage a caseload of members with complex health needs.
• Conduct comprehensive assessments and develop care plans.
• Assess health status, needs and resources.
• Coordinate and implement care plans with providers.
