Job summary

Frisbie Memorial Hospital in Rochester, NH seeks a skilled Case Manager to guide patients and families through care transitions, coordinate services, and develop individualized care plans with physicians and the interdisciplinary team.

You will document assessments, discharge planning, and coordinate education for patients and families, working closely with Rehab Program Director and care teams to ensure safe, patient-centered care.

Qualifications

• RN licensure required
• BCLS obtained within 30 days of start date
• Associate degree required

Responsibilities

• Guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans reflecting patient strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team
• Collaborating with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction
