East Arkansas Family Health Center, Inc. is seeking a Registered Nurse Case Manager for our Osceola, Arkansas location. This full-time role coordinates patient-centered care, develops care plans, and connects patients with healthcare and community resources.

You will collaborate with providers within the Patient-Centered Medical Home model and document care in the EMR to ensure continuity and quality outcomes.

Qualifications

• Associate degree or diploma in registered nursing required.
• Minimum two years of Case Manager experience preferred.
• Experience with case management services preferred.

Responsibilities

• Coordinate patient assessments, treatment planning, and follow-up care.
• Develop individualized care plans and monitor progress toward goals.
• Identify preventive care needs and coordinate with providers for services.
