Job summary

Menorah Medical Center in Kansas is seeking a Case Manager to support patients and families through care transitions. You will coordinate services, develop care plans, and collaborate with physicians, nurses, and leaders to ensure safe transitions and high-quality care across the continuum.

The position emphasizes patient advocacy, education coordination, and teamwork within the rehab program and associated departments.

Qualifications

• Must be a licensed RN
• RN diploma or ADN required (Kansas)
• Certification as RN preferred

Responsibilities

• Guide patients and families through program orientation and discharge criteria
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences
• Develop and implement individualized treatment plans reflecting patient strengths and goals
• Complete psychosocial assessments and discharge planning evaluations and communicate needs across care team
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction
