Job summary

Brown University Health is seeking a Per Diem Case Manager who supports Surgical Services Case Management, Outpatient High Risk Case Management, Transfer & Access, and Discharge Review. The role includes coordinating care, assessing patients, and facilitating post-acute transitions across the health system.

The ideal candidate holds an RN license in RI and MA, with 3–5 years of clinical/case management experience and a preference for case management certification.

Qualifications

• Three to five years of clinical/case management experience preferred.
• Licensure as Registered Nurse in Rhode Island and Massachusetts.
• Certification in case management preferred.

Responsibilities

• Collaborate with departments to coordinate services for patients across the care continuum.
• Perform initial and ongoing case management assessments and coordinate discharge planning.
• Support various programs within Case Management Special Programs (Surgical Services, Outpatient High Risk, Transfer & Access, Discharge Review).
• Assist with information gathering, CQI activities, and ensure timely access to care.
