Heart to Heart Hospice is seeking a dedicated RN Case Manager to support patients and families in Memphis. The role emphasizes comprehensive assessments, care coordination, and adherence to hospice protocols within a multidisciplinary team.

Responsibilities include implementing patient-specific plans of care, collaborating with physicians and IDG members, and ensuring timely documentation. A valid RN license and reliable transportation are required.

Competenze

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing and hold a current RN license.
Minimum two years of RN experience, preferably in a healthcare/clinical setting.
Valid driver’s license and reliable transportation with ongoing auto liability insurance.

Mansioni

Conduct initial and ongoing comprehensive assessments of patients to determine physical, psychosocial, environmental needs and ADLs.
Implement and revise the individualized POC in collaboration with the care team.
Coordinate care among patient, caregivers, physicians and healthcare professionals.
Attend IDG meetings and contribute to care planning and prioritization of patient needs.
Perform on-call duties and provide on-call service as assigned.
Ensure timely submission of all documentation and compliance with laws and policies.
Fulfill other duties as required.
