Job summary

WelbeHealth in Rosemead, CA is seeking a Case Manager, RN to guide participant care within the WelbeHealth PACE interdisciplinary team. You will conduct clinic nursing assessments, develop personalized care plans, and document interventions, while coordinating with the IDT and families to adjust plans as needed.

This weekday role offers a stable Monday–Friday schedule with no weekends or on-call. We provide comprehensive benefits, including medical/dental/vision, 17 days PTO, 12 holidays, 6 sick

Qualifications

• Graduate of an accredited school of nursing.
• California RN license required.
• BLS/First Aid certification required.
• Two years of nursing experience with frail/elderly population preferred.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct clinic nursing assessments to determine participant needs and goals, and participate in IDT meetings to develop care plans.
• Deliver and document nursing interventions as per participants’ care plans, maintain safe environments, respond to physician orders, and administer medications.
• Collaborate with the WelbeHealth PACE Physician and IDT to coordinate care needs and communicate with participants and families to revise plans.
