Job summary

WelbeHealth is seeking a Case Manager, RN in San Jose, CA to guide participant care within the interdisciplinary team. You will conduct nursing assessments, develop personalized care plans, and oversee care transitions, while coordinating with physicians and families.

The role emphasizes relationship-building with participants, a consistent Monday–Friday schedule with no weekend or on-call requirements, and a comprehensive benefits package including 401(k) match and bonus opportunities.

Qualifications

• RN licensed in California (unencumbered).
• BSN preferred.
• Current BLS/First Aid certification required.
• Two years nursing experience with frail/elderly population preferred.
• CPR and First Aid certification within 180 days of hire; maintenance thereafter.

Responsibilities

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals and participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions as per care plans, maintaining safety, responding to physician orders, and administering medications.
• Collaborate with WelbeHealth PACE Physician and IDT to coordinate care needs and communicate with participants and families to revise care plans.
• Instruct participants, families, and caregivers regarding disease process, self-care techniques, and prevention strategies.
