WelbeHealth in California seeks a Case Manager, RN to guide participant care as a key voice on the interdisciplinary team. You will conduct nursing assessments, develop personalized care plans, and support transitions of care coordination with the IDT.

The role features a Monday–Friday schedule with no weekends or on-call, plus a comprehensive package including base pay and bonus, medical/dental/vision, PTO, holidays, 401(k) match, and additional benefits.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Unencumbered California RN License.
• Current BLS/First Aid Certification required.

Responsabilités

• Conduct clinic nursing assessments to determine participant needs and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions per care plans; ensure safe environment and correct meds.
• Instruct participants and caregivers on disease processes, self-care, and prevention strategies.
