Job summary

Bethesda in Willmar, MN is seeking a Case Manager to oversee nursing unit operations, ensuring care plans and the Medical Plan of Care align with state and federal regulations. The role acts as the nursing liaison to residents, families, physicians and the interdisciplinary team.

You will supervise RNs, LPNs, TMAs and CNAs, coordinate resident care, participate in MDS processes, and support quality assurance and staff development. 40 hours/week with weekend/holiday rotation.

Qualifications

• Must be a MN-licensed RN.
• Minimum two years long-term care experience preferred.
• Strong knowledge of Medicare and State/Federal regulations.
• Experience with MDS process and care planning.
• Excellent communication and organizational skills.

Responsibilities

• Maintains unit readiness to meet State and Federal regulations and facility standards.
• Directs, monitors, and assists staff to prevent decline in resident function.
• Monitors quality of resident care and provides guidance for improvement.
• Serves as a resource on resident care, clinical practice, and facility policy.
• Initiates disciplinary action as appropriate and reports to DON/ADON.
• Assists with performance evaluations and continuing education.
