The RN will assess admissions, coordinate with physicians and staff, and supervise Home Health Aides. Flexibility for on-call coverage and local travel is required to meet client needs.

Qualifications

• Education: Graduate of an accredited school of nursing and current Minnesota RN registration.
• Experience: Home Health preferred with geriatric focus; strong nursing skills and basic computer use.
• Personal: Excellent communication, time management, attention to detail, adaptability and confidentiality.

Responsibilities

• Provide, coordinate, and monitor health care services for agency clients.
• Ensure safe delivery of quality care and regulatory compliance.
• Coordinate client care, assess admissions, transfers and current clients.
• Maintain timely documentation and communication with doctors, families, and staff.
• Supervise Home Health Aides and Homemakers; participate in supervisory visits.
• Provide on-call nursing services on a rotating basis.
• Promote agency services to discharge planners, physicians, and families.
