Job summary

Good Shepherd Skilled Nursing is seeking an experienced Registered Nurse (RN) Case Manager to lead a resident-centered household and support staff in delivering high-quality care. You will oversee medication administration, care planning, and timely admissions, discharges, and transfers, ensuring compliance with nursing standards and physician orders.

You will provide supervision, mentor staff, and maintain safety through accurate documentation and ongoing assessment of residents’ needs,

Qualifications

• Current Registered Nurse (RN) license to practice in Minnesota.
• BLS certification including CPR and AED.
• Minimum of two years nursing experience; long-term care experience preferred.
• Demonstrated leadership to develop, motivate, and guide staff; strong critical-thinking and problem-solving skills.

Responsibilities

• Oversee and manage an assigned nursing home household with person-centered care.
• Guide and support household staff in meeting residents' physical, emotional, and psychosocial needs.
• Administer medications and treatments per physician orders and facility policies.
• Coordinate admissions, discharges, and transfers for residents.
