Job summary

Good Samaritan Society is seeking a Registered Nurse Case Manager for our Long Term Care unit in Luverne, MN. The role focuses on assessments, care coordination, and ensuring timely medication administration with documentation.

Responsibilities include developing service plans, educating residents and families, and collaborating with the care team to improve outcomes while maintaining privacy and dignity. Day hours with weekend shifts, on-call rotation, and potential evenings apply.

Qualifications

• Bachelor's degree in Nursing from an accredited program.
• Unencumbered RN license in the state of practice, or multistate licensure under compact.
• Minimum of two years’ clinical experience preferred.

Responsibilities

• Assess, implement and coordinate a plan of care for LTC residents.
• Ensure medications and treatments are administered and documented.
• Educate residents, families and care team; coordinate with physicians and staff.
