Job summary

Kaiser Permanente is seeking an RN Case Manager Liaison Nurse for inpatient discharge planning, onsite at Seattle. The role involves coordinating care across disciplines, advocating for patients, and ensuring timely, effective discharge while managing complex cases.

Requires RN license (WA or compact), Case Manager certification within 36 months, and BLS. Eligible external hires may receive a $5,000 sign-on bonus per policy. This is an onsite, Mon-Fri role with every-other-weekend rotation.

Qualifications

• Minimum three (3) years of recent RN medical/surgical/ambulatory clinical experience.
• Minimum two (2) years of RN experience in ambulatory case management, care coordination or disease management.
• Successful completion of Case Manager Certificate within 36 months of hire.

Responsibilities

• Assesses referred patients to determine physical, mental, financial, psychosocial needs and develops patient-centered plans with time-bound goals.
• Collaborates with other health care professionals on plan of care and communicates progress to care team.
• Directs care planning to ensure seamless continuum of care and coordinates referrals for home health, long term care, hospice, or other services.
