Job summary

Kaiser Permanente is seeking an RN Case Manager Liaison for the Advanced Urgent Care on-site at KPWA Bellevue Medical Center. The role involves coordinating discharge planning and follow-up care for hospitalized KFHPW patients, with a rotating schedule including weekends and holidays.

Responsibilities include collaborating with primary and specialty care teams, maintaining records, and ensuring adherence to care plans while optimizing outcomes.

Qualifications

• Minimum three (3) years of recent RN medical/surgical/ambulatory clinical experience.
• Minimum two (2) years of RN experience in ambulatory case management, care coordination or disease management.
• Registered Nurse License (Washington) or Compact License required at hire.
• Basic Life Support required at hire.
• Case Manager Certificate within 36 months of hire.

Responsibilities

• Plan discharges and follow-up care for KFHPW patients in hospital/outpatient settings.
• Serve as liaison across KFHPW care continuum and with external providers.
• Collect data and provide input to leadership on utilization, cost, quality and care delivery.
• Collaborate with care teams to ensure adherence to the medical plan and improved outcomes.
