Job summary

Kaiser Permanente is seeking an experienced Case Manager Liaison Nurse for Emergency Room Discharge Planning, onsite at KPWA Central Hospital ER, Capitol Hill. The role operates Mon-Fri 10:00 AM–10:30 PM with every 3rd weekend and alternating holidays.

The incumbent will assess patients, develop and implement patient-centered discharge plans, coordinate care with the care team, and advocate for patients while ensuring smooth transitions across settings. A valid RN license and BLS are required.

Qualifications

• Minimum three (3) years of RN medical/surgical/ambulatory clinical experience.
• Minimum two (2) years of RN experience in ambulatory case management, care coordination or disease management.

Responsibilities

• Assess referred patients and develop patient-centered case management plans with goals and timelines.
• Collaborate with health care professionals to plan care and monitor outcomes.
• Coordinate referrals to home health, long-term care, hospice, and other services.
