Job summary

Alternate Solutions Home Health Columbus seeks a Registered Nurse Case Manager to lead patient assessments and develop care plans, coordinating care across disciplines to ensure safe, cost-effective services.

You will educate patients and families, document comprehensively, and work under physician direction. An active RN license is required with at least one year of acute care experience; home health is a plus.

Qualifications

• Minimum of one year of RN experience in acute care; home health experience desired.
• Effective communication with patients, families, physicians and co-workers.
• Calm under pressure with compassionate patient care.
