Job summary

The Waters is seeking an RN Case Manager to support the Director of Health and Wellbeing and oversee daily clinical operations within the community in Edina, MN. You will coordinate nursing care, ensure high-quality resident services, and serve as a key liaison for residents, families, and care providers.

Ideal candidates are licensed RNs with dementia certification, at least two years of geriatric nursing experience, and prior supervisory responsibilities.

Qualifications

• Licensed RN in good standing
• Dementia certification or willingness to obtain it
• Two years of geriatric nursing experience preferred
• Supervisory/leadership experience preferred

Responsibilities

• Plan, organize, and coordinate nursing care for residents.
• Collaborate with physicians, therapists, and providers to update care plans.
• Identify and address education needs beyond routine requirements.
• Conduct comprehensive resident assessments to support person-centered care.
• Oversee delegated services and supervise nursing staff.
• Maintain accurate clinical documentation in data-based systems.
• Respond to emergencies and unexpected situations promptly.
