Qualifications

• Bachelor's degree in Nursing or a related field.
• Minimum 3 years of experience in nursing, case management, or a directly related field.
• Active Georgia RN license required at hire.
• Strong clinical assessment, communication, and organizational skills.
• Ability to collaborate effectively with interdisciplinary healthcare teams.

Responsibilities

• Conduct comprehensive patient and caregiver assessments.
• Develop individualized case management plans and treatment goals.
• Coordinate discharge planning and patient transitions between healthcare settings.
• Assess medical necessity and appropriate levels of care.
• Review eligibility, benefits, referrals, and authorization requirements.
• Collaborate with physicians, nurses, social workers, and multidisciplinary teams.
• Identify barriers to care and connect patients with appropriate healthcare and community resources.
• Monitor patient progress and adjust care plans as needed.
• Maintain accurate clinical documentation and support quality standards.
• Promote patient safety, regulatory compliance, and continuity of care.
