Job summary

Qualifications

• Requires RN licensure and NH eligibility.

Responsibilities

• Determine patient needs and initiate discharge planning within 1–2 business days of admission.
• Maintain communication with UM coordinator regarding assessments and projected lengths of stay.
• Collaborate with physicians and healthcare team to evaluate plan and modify as needed.
• Coordinate transfers and secure authorizations for required services.
• Document assessments and interventions per current standards and policies.
