You will perform thorough assessments and facilitate communication among patients, families, and healthcare providers.

You will develop individualized care plans with interdisciplinary teams, coordinate transitions, maintain accurate documentation, and ensure compliance with healthcare regulations.

Qualifications

• RN license issued by Virginia.
• Experience in case management or intake preferred.
• Proficient with EMR and MS Office.
• Valid driver’s license and reliable transportation.

Requirements

• Current Registered Nurse (RN) license in the state of Virginia.
• Experience in case management, utilization review, or nursing intake services preferred.
• Strong organizational, communication, and interpersonal skills.
• Ability to work independently and as part of a collaborative healthcare team.
• Proficiency with electronic medical records (EMR) systems and Microsoft Office.
• Valid driver’s license and reliable transportation.
