AccentCare, Inc. in Mechanicsville, VA, is seeking an RN Case Manager for Home Health to oversee patient care from admission to discharge and to coordinate a multidisciplinary team.

This full-time, on-site role covers Richmond, East Henrico, and surrounding areas, with responsibilities including care-plan management and direct supervision of clinical staff. The position requires an RN license in the operation state, at least 1 year of RN experience, and CPR certification.

Formación

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsabilidades

• Manage patients' care plans from admission through discharge and ensure delivery of quality patient care.
• Direct staff nurses, LPNs, and CHHAs in delivering the plan of care and coordinate interdisciplinary needs.
• Identify interdisciplinary needs and coordinate allied health clinicians.
• Perform initial patient assessment and develop realistic, measurable goals.
• Inform physicians of assessment results and recommended interventions.
• Document assessments, progress, discharge notes, and required paperwork timely.
