Qualifications

• Two years in acute care nursing; or one year with two years case management experience.

Responsibilities

• Meet with patients within 24 hours of admission for initial assessment.
• Coordinate discharge planning with hospitalists and care team.
• Review medical necessity and level of care with hospitalists.
