Job summary

Cedars-Sinai Medical Center in Marina Del Rey, CA is seeking a Registered Nurse Case Manager to coordinate patient care from pre-hospitalization through discharge. You will work with the care team to ensure appropriate care, infection control, and timely discharge planning.

The role requires at least an Associate's degree in Nursing, a CA RN license, and 2 years of acute care nursing; CCM/ACM is preferred. Shift is 8:00 AM to 4:30 PM onsite. Eligible for a $5,000 referral bonus.

Qualifications

• Associate's degree in Nursing required.
• California RN License required.
• 2 years of nursing experience in an acute care setting required.
• Case Management experience 1 year preferred.
• CCM or ACM certification preferred.

Responsibilities

• Evaluates and coordinates patient care from admission to discharge.
• Documents reviews using utilization criteria.
• Provides telephonic reviews for contracted/private patients.
• Educates patients and families on hospitalization and discharge.
• Collaborates with discharge planning to arrange home care and equipment.
• Ensures timely case management reports for regulatory bodies.
• Maintains professional interactions with patient/family and care team.
