Job summary

Cedars-Sinai in Los Angeles, CA, seeks a Case Manager Care Coordinator to oversee patient transitions from admission to discharge, coordinating clinical and supportive services to ensure safe, timely moves to the next level of care. You will collaborate with interdisciplinary teams to build a comprehensive plan addressing medical, psycho-social and financial needs.

The role requires a nursing degree with an RN license and experience in acute care; Master’s in Nursing preferred. BLS required.

Qualifications

• Bachelor’s Degree Nursing - minimum.
• Master’s Degree Nursing - preferred.
• RN State License - Upon Hire minimum Acute nursing experience
• Basic Life Support (BLS) - Upon Hire minimum BLS Required
• Certified Case Manager - 2 years preferred Experience and certification in case management preferred

Responsibilities

• Coordinate care from admission through discharge to ensure a safe, timely transition to the next appropriate level of care.
• Partner with the healthcare team to address clinical, psycho-social and financial needs during the transition of care plan.
