Job summary

Cedars-Sinai Health System is seeking a Registered Nurse Case Manager to oversee hospitalized patients and post-discharge care coordination. You will assess needs, coordinate care with hospitalists, health plans, and post-acute providers to ensure appropriate utilization and timely discharges.

The role requires an active California RN license, BSN, and 2 years in acute care with case management experience; knowledge of workers' compensation rules and Epic experience is preferred.

Qualifications

• Graduation from an accredited school of nursing is required.
• Bachelor of Science degree in Nursing is required.
• Valid CA RN license required.
• Certified Case Management preferred or willing to obtain within 2 years.

Responsibilities

• Meets with patients within 24 hours of admission and conducts an initial assessment.
• Consults with hospitalist each day during rounds regarding disposition planning.
• Uses Milliman and Interqual guidelines for medical necessity and level of care.
• Coordinates discharge planning with PCP/hospitalist and specialists.
• Documents authorization and clinical information into Nautilus (Access Express) by discharge date.
