Job summary

Cooperative Home Care in St. Louis is seeking an experienced RN Case Manager to provide clinical oversight for our In-Home Care and Private Duty Nursing programs. You will lead a team and ensure high-quality patient care.

The role requires an active RN license with ventilator, trach, and G-tube experience, preferably with pediatrics, and willingness to travel across the St. Louis metro area. After-hours or weekend training shifts may be required.

Qualifications

• Active RN license required.
• Ventilator, trach, and G-tube experience preferred, pediatrics a plus.
• Willingness to travel across the St. Louis metro area.
• Occasional after-hours or weekend training shifts may be required.

Responsibilities

• Perform supervisory visits on LPNs and aides.
• Perform med setups, general health evaluations, and other in-home nursing visits.
• Train PDN nurses on proper care techniques and charting in the home and office.
• Teach CPR, L1ma, and other training classes in office. We will pay you to become an instructor.
• Complete necessary clinical documentation related to patient care.
