Job summary

Cooperative Home Care, Inc. is seeking an experienced RN Case Manager to provide clinical oversight for our In-Home Care and Private Duty Nursing programs. You will lead a team of LPNs and aides, ensure high-quality patient care, and coordinate care plans across settings.

Qualifications

• Valid RN license is required.
• Ventilator, Trach, and G-tube experience preferred (pediatrics encouraged).
• Willingness to drive anywhere in the St. Louis metro area.
• Occasional after-hours or weekend training shifts may be required.

Responsibilities

• Perform supervisory visits on LPNs and aides.
• Perform med setups and in-home nursing visits.
• Train PDN nurses on care techniques and charting.
• Teach CPR, L1ma, and other training classes in-office; employer may fund instructor certs.
• Complete clinical documentation related to patient care.

Duties

• Perform supervisory visits on LPN’s and aides
• Perform med setups, general health evaluations, and other In-home nursing visits
• Train PDN nurses on proper care techniques and charting in the home and office
• Teach CPR, L1ma, and other training classes in office. We will pay you to become an instructor
• Complete necessary clinical documentation related to patient care

Requirements

• Valid RN license
• Previous Ventilator, Trach, and G-tube experience, preferably with pediatrics
• Willing to drive anywhere in the St. Louis metro area
• Willing to perform the occasional after hours or weekend training shift
