Job summary

AccentCare Inc. is seeking an RN Case Manager in Omaha, NE to oversee patient care plans from admission through discharge. You will coordinate with staff nurses, LPNs, and CHHAs to deliver the individual plan of care and address interdisciplinary needs.

This is a full-time position with a schedule of Monday–Friday 8:30 a.m. to 5 p.m., on-call duties of one weeknight per week and one weekend per month. Compensation reflects annual pay estimates, not guaranteed, and includes a comprehensive

Qualifications

Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of agency operation.
A minimum of one year of experience as an RN
Current CPR Certification required.

Responsibilities

Assess the status of nursing needs and overall management of the patient’s care plan.
Complete initial patient assessment upon admission, including patient history.
Select nursing diagnoses and establish measurable goals.
Assess all medicines for potential adverse reactions and contraindications.
Inform the physician of assessment results and recommended interventions.
Implement nursing interventions within the Nurse Practice Act.
Re-evaluate patients’ needs as required.
Document assessments, progress, and discharge notes in a timely fashion.
Inform physician of changes in patient condition and needs.
