Job summary

HCA HealthONE is seeking a Registered Nurse Case Manager (PRN) in Colorado. You will support patients and families through care journeys, coordinating services, resources, and care plans with physicians, nurses, and leaders to ensure safe transitions and high-quality care.

The role emphasizes collaboration, care coordination, and adherence to compassionate care principles within the Rehab Program, with eligibility for comprehensive benefits and supportive workplace culture.

Qualifications

• Registered nurse with an active license (RN) and appropriate degree or diploma.
• Experience in acute care settings and care coordination.
• Ability to work in a fast-paced, multidisciplinary environment.

Responsibilities

• Guide patients/families through orientation, rehab philosophy, and discharge criteria.
• Coordinate education for patients, families, and caregivers.
• Develop and implement individualized treatment plans.
• Complete psychosocial assessments and communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team.
