HCA HealthONE Mountain Ridge is hiring a Registered Nurse Case Manager (PRN) to support patients and families through care transitions. You will coordinate services, develop care plans, and partner with physicians, nurses, and leaders to ensure safe transitions and consistent care quality.

Formación

• RN must be licensed to practice.
• Minimum 2 years in case management or 3 years in acute care.

Responsabilidades

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients and families involved in treatment planning.
• Develop individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and document findings for the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote HCA Healthcare values through every patient interaction.
