Boost Home Healthcare in Greeley, Colorado is hiring an experienced Registered Nurse Case Manager to lead in-home care, manage your own client caseload, guide a team of caregivers, and build lasting relationships with clients and families.

Qualifications

• Active, unencumbered Nursing License in Colorado.
• At least 1 year of home care or home health nursing experience.
• Case management or caregiver supervision experience preferred.
• Current BLS/CPR certification.

Responsibilities

• Complete in-home nursing assessments and initial intake.
• Develop and update individualized plans of care; ensure caregivers follow them.
• Oversee a caseload of home care clients and adjust services as needed.
• Serve as the main clinical contact for families, physicians, and partners; make referrals when needed.
• Maintain accurate, timely client records and comply with regulations (HIPAA).
• Provide on-call support to resolve client and caregiver concerns.
