Full-time, part-time, and per diem options. Ideal candidates will hold a BSN or higher, have an active RN license, 1+ year of case management experience in a healthcare setting, and be proficient with EHR systems. Excellent communication with patients, families, and providers is essential.

Qualifications

• BSN or higher from an accredited program.
• Active, unencumbered RN license.
• 1+ year case management experience in healthcare or hospital setting.
• Strong interpersonal and communication skills for collaboration with medical teams, patients, and families.
• Proficiency with electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive assessments of patients, including medical history, medications, treatment plans, and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans.
• Monitor and evaluate patient progress, adjusting care plans as needed and communicating changes to the healthcare team.
• Serve as the main point of contact between the patient and healthcare providers, ensuring seamless communication and care coordination.
• Maintain up-to-date records and case notes, adhering to confidentiality and compliance standards.
