Responsibilities

• Proactively communicate with payors to secure approvals and resources.
• Maintain knowledge of clinical, system, and fiscal data for patient populations.
• Collaborate with multidisciplinary teams, patients, and families for effective transitions.
• Coordinate transfers to post-acute settings including facilities and rehab.
• Enter and maintain patient information in departmental systems per guidelines
