Qualifications

• Bachelor's Degree BSN (Required).
• Minimum 2 years of clinical nursing experience. One year in case management, discharge planning or utilization review preferred.
• Valid state license as a registered professional nurse (required).
• CPR- Basic CPR (required).
• Certification in case management by a national accreditation organization (ACM – RN) / Certified Case Manager (CCM), preferred.

Responsibilities

• Coordinate patient-centered care across the continuum in collaboration with patients, families, social workers, physicians, and the interdisciplinary team.
• Lead and advocate for transition planning and resource management from pre-admission through post-discharge.
