Job summary

Elara Caring is hiring a Registered Nurse Case Manager in Kansas to support patients in their homes and facilities across Topeka and Lawrence. You will assess needs, coordinate with the care team, and educate families on the hospice plan of care.

The role requires an active RN license, 1+ year of clinical nursing experience, and willingness to travel between patient homes in the assigned area. Experience in hospice or home health is preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing school (Certificate/Diploma/Associate/Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate changes to physician and care team.
• Educate patients, families, and staff on the Plan of Care and hospice philosophy.
