Job summary

Elara Caring is seeking a Clinical Team Manager - Hospice to lead a compassionate care team in the Kansas area. You will oversee patient care services, coordinate referrals, and ensure timely initiation of care according to regulatory guidelines.

You will report to Administration or the Branch Director, support performance metrics, and travel as needed to support patient needs. A nursing degree and RN license are required.

Qualifications

• Associate degree in Nursing or related field required; Bachelor preferred.
• 2 years of home care experience in Home Health or Hospice.
• 1 year of supervisory experience required.
• Current, unrestricted RN license valid for the state of work.
• Willingness to travel up to 50% to support business and patient needs.

Responsibilities

• Provides oversight of all patient care services and personnel including assignments, coordinating patient care, referrals, and developing individualized patient plans.
• Plans, directs, and participates in delivery of home health services with providers, vendors, or facilities.
• Ensures timely start of patient care within regulatory guidelines.
• Coordinates physician orders, staff assignments, and communications among care team, patient, family, and physician.
• Reviews and approves plan of care and evaluates changes for clinical appropriateness.
• Coordinates and integrates orders from all physicians involved in the plan of care.
• Conducts regular OASIS reviews for all patients.
• Mentors and coaches direct care staff; initiates hiring and interviews candidates.
• Communicates performance metrics with Branch Director or Administrator.
• Available for field work as needed.
