Elara Caring in Topeka, KS seeks a Registered Nurse Home Health to assess patients, deliver care per plan, and educate families. You will monitor conditions, report changes, and help ensure safe, high-quality home-based nursing.

Requirements include a current RN license in Kansas, graduation from an accredited nursing program, 1+ year of clinical nursing experience, and ability to travel between patient homes with reliable transportation.

Qualifications

• Current State license as a Registered Nurse (RN).
• Graduate of an accredited nursing program (Certificate, Diploma, ADN or BSN).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to lift, push, pull and assist up to 50 pounds, including patient transfers and equipment.
• Valid driver's license or reliable transportation to travel between patient homes.

Responsabilités

• Assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy per HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on their care and safety.
• Take action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
