St. Croix Hospice in Wentzville, MO is seeking a compassionate Registered Nurse Case Manager to deliver in-home, holistic hospice care.

You will collaborate with a care team to develop personalized plans, lead end-of-life conversations, and ensure symptom management with empathy and clinical excellence. The role requires RN licensure and 1+ year acute care experience; hospice/home health experience is preferred.

Competenze

• RN licensed with ADN/BSN/MSN required.
• 1+ years of acute care experience required.
• Hospice/home health experience preferred.
• Perform initial and ongoing patient assessments.
• Develop and update individualized care plans.

Mansioni

• Deliver in-home, holistic hospice care with empathy.
