Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide 1:1 holistic care to patients in their homes.

You will work with an interdisciplinary team to develop personalized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy. Requirements include an RN license and 1+ year acute care experience; hospice/home health experience is preferred.

Qualifications

• RN licensure required.
• 1+ year acute care experience.
• Hospice/home health experience preferred.
• Ability to develop and adjust individualized care plans.

Responsibilities

• Develop individualized care plans with patient and family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and ensure smooth care transitions.
• Perform comprehensive assessments to determine hospice needs.
