Job summary

Tapestry Hospice & Palliative Care in North Georgia is hiring a Nurse Case Manager (RNCM). You will plan and direct hospice care using the nursing process, collaborating with the Interdisciplinary Team to support patients and families with dignity and respect.

Qualifications

• Current RN license in the State of Georgia.
• 1–2 years of nursing experience (preferred).
• 1–2 years of hospice experience preferred; candidates with experience in skilled nursing, home health, acute care, med-surg, or ICU will be considered.
• Utmost compassion and integrity.
• Valid driver’s license, insurance, and reliable vehicle.
• Excellent time management skills.

Responsibilities

• Create, plan, and direct hospice care by utilizing the nursing process of assessment, intervention, implementation, and evaluation.
