Job summary

Tapestry Hospice & Palliative Care in North Georgia is seeking a Nurse Case Manager (RNCM) to create, plan, and direct hospice care using the nursing process. You will collaborate with the Interdisciplinary Team and build meaningful relationships with patients and families, upholding the highest standards of care and professional nursing competency.

• Experience in skilled nursing, home health, acute care, med-surg, or ICU will be considered.

• Valid driver’s license, insurance, and reliable vehicle.
• Excellent time management skills.

Position Description

As a Nurse Case Manager (RNCM), you will create, plan, and direct hospice care by utilizing the nursing process of assessment, intervention, implementation, and evaluation. This role requires a commitment to the highest standards of care and professional nursing clinical competency.

Minimum Requirements

• Current RN license in the State of Georgia
• 1–2 years of nursing experience (preferred)
• 1–2 years of hospice experience preferred
