Job summary

AMN Healthcare is seeking an RN Case Manager – Hospice for a 13-week assignment in Sunnyvale, CA, starting ASAP. The role involves admitting patients, assessing needs, and creating an individualized plan of care in coordination with a multidisciplinary team.

Must hold a CA RN license and BLS, with hospice case management experience. Shifts Sun-Thu 10am-7pm or 11am-8pm, with schedule flexibility based on patient availability.

Responsibilities

POSITION SUMMARY – RN Case Manager – Hospice

POSITION DUTIES

Admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.

Minimum Required Qualifications –

• CA RN license
• BLS
• 1+ year hospice case management experience

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

Sun-Thu 10am-7pm or 11am-8pm (Schedule can be flexible based on patient's availability)
