Job summary

Career Strategies is seeking a Hospice RN Case Manager in San Diego, CA to join our dedicated team. This Full Time role delivers patient-centered end-of-life care and coordinates care plans and assessments.

The ideal candidate holds a CA RN license, CPR certification, and a valid driver’s license, with hospice or home health experience preferred.

You will work daytime shifts, document care notes, and collaborate with families to support comfort and dignity throughout the patient journey.

Qualifications

• Active RN license in CA.
• Hospice or home health experience preferred.
• Valid driver’s license.
• Current CPR certification.
• Excellent observation, verbal, and written communication skills.

Responsibilities

• Create and initiate care plans through assessing the history of current and previous illnesses.
• Spend quality one-on-one time with patients and their families to support everyday needs.
• Care for patients by providing initial and ongoing assessments of patient and family needs.
• Record important health information including temperature, pulse, respiration, and blood pressure.
• Administer medications and treatments as prescribed by the physician.
• Provide health care instructions to the patient as appropriate.
• Record symptom management changes and outcomes.
• Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given.
• And additional duties as assigned

Qualifications On An Average Day You Will:

• An RN license in good standing in CA.
• 1 year of experience in hospice or home health preferred
• A valid driver’s license
• Current CPR certification
• Excellent observation, verbal, and written communication skills
• And additional related duties as needed
