Job summary

Moments Hospice in Madison, WI is seeking an experienced RN Case Manager to lead clinical care for patients and families, coordinating symptoms, care plans, and end-of-life support.

This full-time, field-based role offers a fleet car program, comprehensive health/dental/vision insurance, life and disability coverage, generous PTO, and 401(k) with company match. Schedule is Monday–Friday, 8:30 AM–5:00 PM; salary $75k–$90k.

Qualifications

• Active RN license in the state of work.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across your caseload and act as the primary liaison with patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
