Job summary

Moments Hospice in the United States is seeking an experienced RN Case Manager to lead patient care in the field. You will manage symptoms, coordinate services, and partner with families to support comfort and dignity at end of life.

This full-time, Monday–Friday role offers direct access to leadership, reasonable caseloads, and a fleet car program. You will be the clinical lead across your caseload, guiding care decisions and educating families throughout the process.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational and communication skills.
• Ability to work independently with high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
