Job summary

Moments Hospice is seeking an RN Case Manager to lead clinical care for a portfolio of patients in a field-based role. You will manage symptoms, coordinate care, and guide families through end-of-life processes with a compassionate, patient-centered approach.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
